The ‘Complex Needs in Primary Care’ award will be presented to an exceptional team that can demonstrate delivery of high quality, compassionate care for people with complex needs in primary care settings. The number of people living in the community with complex, multiple heath conditions is increasing, creating a growing demand for primary health care services. In 2019 The Burdett Trust ran a grants programme focused on this topic.
This award will recognise the excellence of multi-disciplinary and multi-agency nursing teams, who through coordinated, person-centred and preventive care are reducing unnecessary hospital admissions and enabling patients with complex needs to live fulfilled lives at home, or closer to home, with reduced reliance on traditional hospital settings.
